We are a social services company that provides direct care services to adults with developmental disabilities. We currently provide quality services in twenty plus home sites throughout Franklin & Delaware County. The job duties vary due to individual client(s) needs. Job Responsibilities:
  • Provide service delivery in compliance with the Individual Service Plans (ISP) and company procedures.  This includes, but is not limited to:  medication monitoring, home cleaning, helping the consumer(s) with proper dress, level of hygiene, documentation of shift responsibilities/activities, documenting descriptions of hours of personal care (HPC records), communication with the consumer, their support person(s) and treatment team.
  • Ensures the health & safety of the consumer(s) are maintained through administration of medications according to Delegated Nursing guidelines, continuous monitoring of consumer and their environment.
  • Communicates and works well with the consumer(s) and their advocates, MRCS peers, supervisors and the community.  Timely & accurate/clear in all oral & written communication of both routine & unusual matters.
  • Provides & facilitates transportation with consumers social, medical & general developmental needs.
  • Encourages & assists the consumer(s) with enhancement of daily living skills.
  • Provides regular input regarding the consumers needs/accomplishments to the Home Manager & the ISP team.
  • Maintains consumer records & other documentation according to established quality guidelines.
  • Reports all Unusual Incidents & Major Unusual Incidents (MUI's) according to company, FCBDD & state guidelines as presented during employee orientation & ongoing training.
  • Ensure cleanliness of the consumer's home & monitor overall service delivery.

Job Qualifications:
  • Current & valid State of Ohio drivers license (fewer than 6 points), reliable car & proof of current auto insurance.
  • Must be at least 18 years of age & have a high school diploma or GED.
  • Good oral & written communication skills.  Ability to work successfully in a team environment.
  • Ability to followed detailed information accurately & document all areas of service delivery according to State, County & company expectations & requirements.
  • Ability to apply good, mature decision making skills within prescribed limits.
  • Required to be reliable & work according to the home monthly schedule.  Arrive on-time for shifts & document time accurately.
  • Sincere desire to enhance the lives of the consumers that the company serves.
  • Adult CPR/First Aid/AED & Delegated Nursing (annual refresher courses provided).
